- MCS-7825-I2 & IBM x306m, BIOS 1.42 & 1.43 error prevents a system from booting if rebooted after October 1, 2009
A defect in the HostRAID BIOS 2106 (included with System BIOS version 1.42 and 1.43) will prevent a server from booting successfully if the server is rebooted after October 1, 2009. This is caused by a defect in the IBM BIOS and is documented in IBM RETAIN Tip: H182636 - Nexus 7000 May Encounter an Unexpected Supervisor Switchover or Reload Due to High Availability Policy for Diagnostic Service
Two specific diagnostic tests on the Nexus 7000 can cause an unexpected process error due to a slow resource leak resulting in two scenarios: 1) A supervisor switchover due to the high availability (HA) policy for service diag_port_lb, when dual supervisors are present - GSR PRP-1 and PRP-2 – Unable To Read Compact Flash Disk and/or ATA Flash Disk (1 GB CF, 1GB FD) – Data Loss, System Boot Hang – Replace Flash Card – Workaround Fix-on failure
The PCMCIA ATA and Compact Flash Disks used in specific Cisco products may contain a Manufacturing defect which reduces reliability of the Flash device. - GSR PRP-1 and PRP-2 – Unable To Read PCMCIA ATA Flash (64M, 128M, 256M, 512M) – Data Loss or System Boot Hang – Replace PCMCIA ATA Flash- Workaround Fix on Failure
The PCMCIA ATA Flash Disks used in specific Cisco products may contain a Manufacturing defect which reduces reliability of the Flash device. PRP-1 and PRP-2 cards used in the Cisco GSR platforms that may contain a suspect Flash device may not be able to access stored files on disk 0/1. - Field Notice: FN – 63255 – MCS-7825-I2 & IBM x306m, BIOS 1.42 & 1.43 error prevents a system from booting if rebooted after October 1, 2009 [Cisco 7800 Series Media Convergence Servers] – Cisco Systems

A defect exists on the stacking connector. The cable connector on the blade products listed in the Products Affected section can break after a few insertions. (CCO login required) - Field Notice: FN - 63275 - MDS NX-OS 4.2(1) Software Deferral due to two software bugs - Upgrade Required [Cisco MDS 9000 Series Multilayer Switches] – Cisco Systems
Two problems have been identified in MDS 4.2(1).The first issue occurs when installing the SSI image onto a SSM using the install ssi command. The port manager process crashes, which causes a failover to occur. The port manager process continues to crash, which causes the switch to reload.
The second issue occurs in some cases of zone change where unwanted TCAM entries can still persist and the member does not receive change notification. (CCO login required)
- Cisco Computer Telephony Integration Object Server (CTIOS) CTIOS Server Assertions
During high call volume in production, CTIOS may restart or assert with a minidump, failure to accept new client connections due to previous exceptions that may cause agent login failure, button enablement issues related agent state or call control. If CTIOS Server node did not restart automatically, it may need to be cycled via Cisco ICM Service Control to recover from this condition.
